Reenrollment Time Period: Wednesday, November 6 (12 p.m. ET) through Friday, November 22 (7 p.m. ET)

Am I eligible to apply for reenrollment?
You are eligible to apply for 2025 disease program participation if you:
- are actively enrolled in an eligible purple disease program;
- are a U.S. citizen or permanent resident;
- have been diagnosed with the disease state for which you are applying to reenroll; and
- have been prescribed an FDA-approved treatment for the disease and have health insurance that covers the prescribed medication.
How do I apply for reenrollment?
Once the reenrollment window opens, you can apply for reenrollment by using one of the following methods:

If we have your cell phone on file, you may receive your unique reenrollment link by text.
Email:
If we have an email for you on file, TAF will send a reenrollment link to that email address on November 6 at 12 p.m. ET. To confirm that the email address we have for you is up to date, please call (855) 845-3663 to speak with a Patient Advocate.
Paper:
If you wish to complete a paper application, please call (833) 343-2148.
Paper applications must be requested and will not be provided prior to November 6.
