- U.S. citizen or permanent resident living in the US or Puerto Rico
- Diagnosed with the specific disease state for which you are reenrolling
- Prescribed an FDA approved medication for the disease state
- Have prescription drug coverage for the prescribed medication

When does 2019 Reenrollment open?
2019 Reenrollment opens October 15 9am EST - November 16, 2018 6pm EST.
How do I Reenroll?
You may reenroll online or submit a paper application. If we have your cell phone and/or email address on file, you will receive a text message and/or email with your personalized link for the Reenrollment portal on October 15. If reenrollment is open, you can also select the "Reenroll Now!!" button to the right. The fastest way to reenroll is by utilizing one of the above methods, however, if you would like a paper application you can print one by selecting the option to the right or by calling (855) 845-3663 and selecting the Reenrollment self-service menu. You may submit the paper application back to us by mail, fax, or by selecting the “Upload Now!” button to the right.
Where do I find my activation code?
If you are reenrolling online, you will be required to input your activation code. Your activation code was mailed to you. You may also obtain your activation code by calling (855) 845-3663 and selecting the Reenrollment self-service menu.
How do I check my Reenrollment Application Status?
You may check your Reenrollment Application status by calling (855) 845-3663 and selecting the Reenrollment self-service menu. You will need your activation code to access your status.
How do I check if I have been enrolled for 2019 program participation?
You will be notified of your 2019 Enrollment status by late January. You may also check your status beginning January 1, 2019 by calling (855) 845-3663 and selecting Option 2 under the Patient menu.
